What happens to all those old old cemetaries?

How do they accomodate all the dead in the crowded old cemetaries?

I would imagine that the bodies are either moved and the site is built over, with the graves being marked in some sort of record, or the cemetery is just built over.Depending on how old it is the bodies in  the coffins would be so decayed that all that would be left are the bones.
