Does any one know how to make a PDF form have editable fields?

Is there a software that can make a PDF form have editable fields. I am VP of sales for a company and I am needing some order forms that my sales reps can edit to process the order. Right now we print and hand write the order and it is not very professional. Any free soft would be great!!!! Thank you

Click http://www.onlinedocumentconversion.com/ upload, convert to rtf & then edit in MS Word.

24x7, Paypal, few cents per page (paid sites are better).
